  • Your first visit is about getting to know you. We’ll review your medical history, current concerns, medications, and goals for care. This appointment is typically more in‑depth so we can create a thoughtful, individualized plan.

  • Please bring:

    • A valid photo ID

    • Your insurance card

    • Any relevant medical records or notes from other providers (if applicable)

    • A list of the medications you're currently taking

    • Payment for any co-pays or out-of-pocket expenses


  • Appointments can be scheduled online through Tebra. After scheduling, our team will follow up to ensure required paperwork is completed. Appointment scheduling can be found under each individual provider’s profile.

  • Yes, we offer telehealth services for certain types of appointments.

Getting Started

  • Yes. While most people schedule appointments online; there are still required intake paperwork must be completed before your first visit.

    • We will follow up with you to ensure paperwork is completed.

    • If you are unable to complete it ahead of time without communication appointments may be reschedule. If you need accommodations to submit paperwork, just let us know.

    • Paperwork can be completed in the office—please plan to arrive 20 minutes early

  • Please bring:

  • We allow for 10 minutes late period, however due to timing, being late may result in a shorter duration of an appointment and therefore all concerns may not be addressed at that appointment.

  • We understand life happens.

    If you need to change your appointment, please call us at least 24 hours before to avoid a fee. This lets us give the spot to someone else who needs it.

Paperwork & Check‑In

  • Our team works collaboratively, and we will help match you with the provider who best fits your needs, preferences, and goals for care.

  • No problem at all. If you ever feel another provider may be a better fit, we’re happy to help you make that change.

  • Yes. A full clinical evaluation will be completed at your first visit to assess the safety and appropriateness of continuing, adjusting, or changing medications. Our priority is safe, effective, and individualized care.

Providers & Care Team

  • Follow‑up visits focus on how you’re doing, how your treatment plan is working, and any adjustments that may be needed. These visits are typically shorter and more focused than your first appointment.

  • Yes, if clinically appropriate, we can assist with FMLA or short‑term disability paperwork. Please let us know in advance so we can plan accordingly.

  • If clinically appropriate, labs or referrals may be ordered. This will always be discussed with you.

  • You can access your medical records through our patient portal.

    If you need physical copies or have any issues accessing the portal, please contact our office for assistance.

Follow‑Up & Ongoing Care

  • Yes. A full clinical evaluation will be completed at your first visit to assess the safety and appropriateness of continuing, adjusting, or changing medications. Our priority is safe, effective, and individualized care.

  • When clinically appropriate and safe, controlled substances may be prescribed. This always follows thorough evaluation and monitoring standards.

  • Prescription refills can be requested through the patient portal, by calling our office, or by contacting your pharmacy directly. Please allow 72 hours for the refill to be processed. Refills may require a follow‑up visit, depending on the medication.
